The funding is for expreienced practices who are looking to expand or improve their integration work. The Funding opportunity is for a 3 year period, at up to $200,000 per year. Thanks, Cindy! This tool has generated a lot of interest nationally. The tool was posted on the Group Space that is being used by a HRSA-funded Systems Integration grant that is looking at improving care coordination for children and youth with special health care needs. Twelve states, including MA, are involved in this grant. Also, AHRQ sent out a notice about the tool, and a link to it.
